Appointments are required to apply for financial assistance. They may only be made by calling the Intake Coordinator at 502-447-4330 and following the phone prompts.

Mon-Thurs: 9:00am-12:30pm.
We are closed on Fridays.
Applicants will Need:
- Proof of address in the Shively Area Ministries service area (40216 ZIP code)
- A photo ID
- Social Security cards of all members of your household
Depending upon the assistance needed, we may also require you to provide past-due notice for utilities or housing, an eviction notice, a prescription, or current proof of income.
